You will use some specific QI tools in this assignment, all of which are found in the CIToolkit. 

 

Step 1: Data

In last week’s assignment, you identified data that would substantiate the existence of the problem. You also identified regulatory agencies that would drive the need for improvement on the selected problem. Those agencies may have quality measures that have already been tested and established. These quality measures will tell you what data needs to be collected regarding your quality issue. 

 

Collect appropriate data needed to analyze the problem and establish the severity of the problem compared to established benchmarks or quality indicators. 

 

Measure the performance of the process or system that produces the effect.

 

Clarify why the process or system produces the effect you want to improve. This may involve completing a detailed flowchart.

 

Use QI tools to display the data that will reveal disparities, patterns, or trends. See the Graphical Analysis section of the CIToolkit.

 

Step 2: Cause and Effect

Keep in mind the 3 common mistakes in problem-solving.

Assuming you know what the problem is without actually seeing what is happening
Assuming you know how to fix a problem without finding out what is causing it
Assuming you know what is causing the problem without confirming it

 

For this step, you will need to complete a fishbone diagram. The fishbone diagram is an effective means of determining the cause and effect of an existing problem.

Complete the Fishbone Diagram Template. 

 

Investigate the problem based on the inputs on your fishbone diagram.

 

Determine the cause of the problem.

 

Create a 10- to 12-slide Microsoft® PowerPoint® presentation with detailed speaker notes in which you:

Summarize your analysis of Step 1 and Step 2.
Include an explanation of data collected to identify the problem, your investigation of the possible causes, and your determination of the cause of the problem.
Include your visual representation of data from Step 1 and your fishbone diagram from Step 2 with explanations of each.
Identify the measures you will use to determine project improvement and success. Provide rationale for the measures.
For example, if the issue you are addressing is falls, is there a benchmark your organization should be achieving? What is the benchmark? How is it calculated? Are there quality measures for falls? This might be the measure you will use to determine success.
Explain how you will know a change is an improvement.
