Assessment Stephen is awake, alert and pink Vital signs: • Temperature – afebrile- 36.20€ • Respiratory Rate: 20 bpm • Pulse rate: 78 bpm • BP: 135/75 mmHg • Sp02: 95% on room air (RA) Responsibility Inform the RN (documented nursing Psychological support – visiting psychologist and counsellor in the GP clinic today. intervention plan of Patient education – common signs and symptoms experienced due to lack of oxygen – Identify six (6) abnormal signs and symptoms that Stephan would expect to experience due to lack of oxygen.
