Medication Profile #12

Insulin NPH  12 units SQ BID

Insulin regular 3 units SQ TID AC

Phenytoin 300 mg PO TID

Losartan 25mg PO daily

Lovastatin 20 mg PO QD

Aspirin EC 81 mg PO QD

With each of these medications listed above i need help on how to constructing a MAR with explaining 5 rights of medication administration, i am one of the rights meaning my name will go in as patient so need help on the other 4 criteria, medication name, dose, route, and frequency used and then need to provide rationale for each medication listed above like why did i choose this time of the day like 8 oclock for example to give this medication is it because of  safety or is it because of interactions with other medications listed above dietary restrictions, side effects, improved compliance..
