50 year old African American Male with no Advanced Directives
Unknown past medical history-No medications, has not been to a primary care physician 
in 30+ years. 
Collapsed in the post office, bystander CPR started (no AED used), EMS arrival patient 
in PEA
Patient’s down time-70 minutes, transferred to the ICU
6 days later patient cardiac arrested-PEA-Down time-35 minutes
AssessmentNeurological-unresponsive, does not respond to voice, does not withdraw to pain
Pupils-round, sluggish Left-4, Right 5-Corneal Reflex-Absent
Glasgow Coma Scale-3, RASS Score -5
Respiratory rate-tachypnea and ataxic
Cardiac-heart rate-120’s A-Fib
GI-active bowel sounds in all 4 quadrants, incontinent
GU-Urine-Foley-cloudy, yellow, sediment-30 ml/hr
Integumentary-Pressure Ulcer(stage II) on coccyx
EEG-impression-low voltage brain activity, intermittent myoclonic seizures (seizures are 
not visible to the RN)
MRI-Anoxic Brain Injury
After 2 weeks the patient was not able to be liberated from the ventilator. The patient 
was unresponsive, did not respond to voice or painful stimuli, pupil responsive was 
sluggish and the corneal reflexes were absent. The patient underwent a tracheotomy and 
PEG tube insertion per the request of the family. The patient was transferred to a long-
term patient care facility after a 2 month hospitalization.
