Butch Sampson, age 68, is a veteran of the Vietnam War, where he was exposed to Agent

 

Orange. Butch recently showed up at the VA clinic where he said the

Butch was admitted to the VA hospital . He has Type 2 diabetes . Debridement of 

the right great toe was done . Previous to this two toes were removed . He is a 

smoker , he has uncontrolled diabetes and uncontrolled high blood pressure .

 

Discharging Butch 

 

After 3 days Butch is being discharged to a transitional home  at 1000 am .

Blood pressure 98/64 this morning . It was 186/98 the night before and Hydralazine

was given . Medication : Amoxicillin/ Clavulanate Potassium 500 mg three times daily

For 5 days . Metformin 500 mg twice daily with AM and PM meal .Ibuprofen 600 mg 

oral for foot pain . Metoprolol 50 mg twice daily . Spironolactone 25 mg oral once a day

in the morning .

 

Provider Order 

 

Discharge to transitional housing.

 

2.Arrange for home health visits by nursing and physical therapy .

 

3.Admit to Case Management and assign Social worker .

 

4.Encourage Medium Calorie Carbohydrate Consistent diet

 

5.Limit weight bearing to right foot. Keep foot elevated when possible. Use cane for ambulation.

6.Follow-up appointment in 5 days at Healthcare for Homeless Veterans office

 

7.Keep incision covered with clean dry dressing. Wear post-surgical shoe except when in

bed.
