This week we focused on risk management. Select from the list of risk categories below and discuss an example of a clinical issue you have seen or experienced (please do not reveal the facility or any patient or caregiver names) that would fall under your chosen category.
Risk Categories:
Error: failure to complete a planned action as intended or using a wrong plan.
Adverse event: when the injury is caused by medical management that results in a measurable disability.
Unpreventable adverse event: an adverse event resulting from a complication that could not be prevented given the current state of knowledge.
Near miss: an event that could have resulted in an accident, injury, or illness but did not by chance or timely intervention.
Sentinel event: an unexpected outcome involving death, serious physical or psychological injury, or permanent loss of function unrelated to the natural course of the patient’s illness.
