On 5/5/22 this LPN is assigned to female patient who sustained a right hip fracture from a fall at home and recently had a hip replacement 3 days ago. LPN received patient at beginning of her/his shift at 7:15am and noted this patient awake A/Ox3 laying in supine position on her bed watching TV. She then had breakfast with good appetite at 8:00am. Patient had a sponge bath at 9:00am, assisted to get dressed and was assisted to a wheelchair. At 10:30am, Physical therapist took patient to the gym and returned to her room at 11:45am. Patient complained of right hip pain at 12:00pm 8/10 and was showing facial grimmace while rubbing her right hip. Nurse inspected patient’s incision site on her right hip which was dry and clean with dressing intact with no drainage present. She repositioned patient comfortably in her wheelchair. LPN then administered PRN dose of Norco 5/325 1 tablet p.o. Then LPN came back after 40 mins, patient states she feels better. Patient had no further complaints the rest of the shift.
