What ethical dilemma is the RN presented with?
The nurse faces the dilemma of respect for autonomy and beneficence (Luebbert, 2022). Competent clients have the right to refuse medication either directly or through a proxy. This is also supported by U.S. statutes. Since the client is in a delusional state he is unable to rationalize or make informed decisions and is therefore not competent. There is a tension between autonomy and beneficence because the RN knows that taking these medications for serious medical conditions will be of beneficence to the client. “Treatment should not be withheld for patients who do not understand what they are refusing when the risks of that refusal are high.” (Tunzi & Spike, 2014). Based on the four primary ethical principles, I believe that beneficence, nonmaleficence, and justice outweigh autonomy in this case.Â
