Patient states – He was working and started feeling like his blood sugar was high
He checked his blood sugar to find it at 586. Normally it’s 200.
He took 20 units of his insulin and then “blacked out.” Patient states the next thing he remembers is waking up with a coworker at his side.
Pt states his insulin pump has been working appropriately. Pt also states he recently had an MI requiring two stents.
Pt denies headache, nausea/vomiting, shortness of breath, chest pain, changes in diet or medications
Physical Exam
Physical findings – Patient was conscious, alert, oriented (CAO). No facial droop, no one-sided weakness, and no slurred speech.
12-lead – NSR 2mm ST elevation in leads V3 and V4 with no reciprocal changes
GCS – 15
BP – 180/90
HR – 86
RR – 16
EKG – NSR
CBG – 548
