Case Study Scenario:

Mr David Williams is a 35-year-old Aboriginal and Torres Strait Islander (ATSI) man.  He has attended the local base hospital to address flu-like symptoms.

David lives with his partner in a rental property with their two children and three grandchildren and two adopted children. His partner is trying to find employment and is currently on New Start payments.

 

David has a past history of 

â–º Hypertension

â–º Type 2 Diabetes

► Chronic Kidney Disease 

â–º Osteoarthritis of the knees

 

David had a history of poorly controlled diabetes and difficulty engaging with the primary healthcare centre as he works shift work as a fly-in fly-out (FIFO) plant operator.

For management of his conditions, David is prescribed the following medications; 

Metformin, Irbesartan and Amlodipine, Hydrochlorothiazide, Atorvastatin, and Vitamin D. 

Allocated to David’s care, you start to establish the care planning with the help of treating team.
