Scenario 1: Medication errors

You are a new graduate nurse working in a busy medical ward when the 80-year-old patient in bed two stops you and asks about his medication.  He questions the number of tablets he has been given as he normally only has one “pink” tablet, not two.  You are very busy as there is a woman acutely unwell in the neighboring room and reassure him that the nurse would have administered what had been charted and move on to assist the unwell patient.  Later that evening, you learn that the man in bed two had become unwell, and it was noted that there had been a medication error due to an incorrectly charted medication.  You feel terrible and seek out your nurse in charge for support.  In your discussions with the nurse in charge, you learn that the ward has recorded an increase in medication errors over the last few months. These medication errors are more prevalent in older patients and can have serious adverse patient outcomes. In consultation with the NUM, you decide to look for strategies to reduce medication administration errors on the ward.  The NUM asks you to present your findings to the ward at the next ward meeting.
