Patient’s Interview, need to make my own interview script with regards to admitting patient/s.

This is the criteria: 

Procedure: SIMULATED INTERVIEW

Explains concisely and correctly the purpose of data gathering

Asks appropriate questions in order to elicit accurate data
         
3. Uses words / language appropriate to the informant’s level of understanding
