A 26-year-old man presents with progressive confusion and a reduced level of consciousness. He has recently been diagnosed with pneumonia and is receiving amoxacillin. For the past few days, the patient has been complaining of excessive thirst and urination and sleeps most of the day. His past history is unremarkable. He takes no medications or illicit drugs. He drinks a beer or two most days but has had none recently. Physical examination reveals a young man who is poorly responsive. He is disoriented for time and place. His temperature is 960F, blood pressure is 80/50 mm Hg, pulse is 100 beats/minute, and he is taking deep, rapid breaths, 20/minute. His mucous membranes are dry. There are no abnormal physical findings. Neurologic examination is normal except for the disorientation and somnolence.
