Mrs. Smith has been admitted to the hospital with a urinary tract infection. In addition to his routine medications, he was started on an antibiotic, and pain medication, and last night he was given a sleeping pill because the night nurse was concerned that he was trying to get out of bed. You note that during the afternoon shift, he has a change in mental status. He is very agitated. He has pulled out his IV and indwelling catheter, and he is trying to leave the hospital.
