During data collection her daughter states she was walking short distance with a walker up until her fall. She also tells you she usually has a great appetite but, in the hospital, she didn’t eat much and attributes it to the ground diet she was on there. She reports sha has been living on a yogurt and maybe a bowl of soup a day for the past of couple days. She tells you that she was on bed for the most part of her hospital stays and fells she would eat better if she was out of bed and assisted with cutting her food. She has a weight loss over the last 6 months and refuses to wear her upper denture. She also tells you she has been incontinent for many years due to her advancing dementia and wears an adult diaper at all times.
